We are looking for users for our day-care short term intensive preventive services.

●What is the Outpatient Short-Term Intensive Preventive Service
This service is to prevent the decline of daily functions and develop the ability to manage oneself in order to prevent the need for nursing care as much as possible through meetings with rehabilitation specialists and group programs.

● Target
Those who are 65 years old or older, meet one of the following criteria and can participate in all 12 sessions
Those who are certified by the nursing care insurance as requiring support 1 ・ Those who are certified as requiring support 2
Those who are judged to have declined in daily function by the basic check list and registered as eligible for the comprehensive project

Target area ： Aso ・ Mutsumi area, Takatsu ・ Midorigaoka area, Yachiyodai area
（ Katsutadai area, Murakami area and Owada area have reached capacity, so applications for January start course are closed. 6 years Please contact us for April start course ）

●Program Contents
0Remove concerns about daily life
We will talk about the problems in daily life and the desired life after the service, and together with rehabilitation specialists, we will assess （ the causes of difficulties in daily life Evaluation ・ Analysis We will then propose ways to alleviate the causes of the difficulties. We will suggest ways to alleviate the causes and advise on home exercise programs and environmental modifications to enable you to live with confidence.

z Self-management （ Enables self-management ）
We will give you pointers on how to maintain your health tailored to each individual and how to maintain and live with your body condition after the service ends.


No physical touch treatment will be provided No rehabilitation treatment by rehabilitation specialists will be provided. We will advise you on key points so that you can continue exercising on your own after the service is over.


We support our residents to live their own life in their own community

We support our residents to choose their favorite activities such as hobbies, sports, volunteer activities, employment, and other activities in their own community, and to live in their own community with confidence. ●Inquiries
